Free Security Incident Report Template
Use this when something happens on post: a theft, an injury, a fight, damage, a trespasser or anything out of the ordinary. Fill it in on screen and print it or save it as a PDF, or print it blank.
Tips
- Write it as soon as you can, while the details are fresh. Use the notes you took at the time.
- Stick to facts: what you saw, heard and did. Leave out opinions and guesses.
- Cover who, what, when, where and how. Leave out why unless someone told you; then quote them.
- Use exact times, and put people's exact words in quotation marks.
- Describe people by what you saw: clothing, build, height, and which way they went.
- Write in the first person, in the order things happened.
- Don't erase. Cross out a mistake with one line and initial it.
A general template. Your employer's or client's policy and your state's rules come first. Not legal advice. What you type stays in your browser; nothing is sent to us.
